Sultan Mind Psychiatry — Practice Policies

Please review the following policies before your appointment. These help us provide consistent, high-quality care to every patient.

Medication Policy

At Sultan Mind Psychiatry, medication recommendations are based on a comprehensive psychiatric evaluation and clinical judgment. We do not provide refills for medications prescribed by other providers or medications used for non-psychiatric medical conditions. Medication prescriptions are not guaranteed during an initial evaluation.

Controlled Substance Policy

Controlled medications are prescribed only when clinically appropriate and after careful evaluation. We may prescribe certain controlled medications in limited circumstances, such as specific anxiety-related conditions or short-term sleep concerns. We do not prescribe opioids and generally do not prescribe benzodiazepines for routine or long-term use. Patients prescribed a controlled substance are required to review and sign a Controlled Substance Agreement outlining the terms of treatment.

Document Policy

Requests for paperwork or documentation require an established therapeutic relationship and a clinical discussion regarding functional impairments. Completion of requested forms is based on clinical assessment and appropriateness. Please allow up to 5 business days for processing.

Appointment Cancellation Policy

Appointment times are reserved specifically for each patient. We request a minimum of 48-hour notice for cancellations or rescheduling. Appointments canceled with less than 48-hour notice, missed appointments, or arrivals more than 10 minutes late may be considered a no-show and may result in a fee of up to the full appointment rate or a minimum charge of $75. Insurance companies do not reimburse missed appointment fees.

Medication Refill Policy

Medication refill requests are encouraged during scheduled appointments to allow discussion regarding treatment effectiveness and ongoing care. Refill requests made outside scheduled appointments may require up to 24–48 hours for processing. Medication refills require active participation in treatment and a scheduled follow-up appointment.

Confidentiality Policy

Your privacy is important to us. Information shared during treatment is confidential and protected by law. Information may only be released with your written authorization unless disclosure is required by law, including situations involving suspected abuse, neglect, exploitation, or immediate risk of harm to yourself or others.
